29 CFR 1910.212(a)(1): Machine guarding was not provided to protect operator(s) and other employees from hazard(s) created by point of operation, in-running point and rotating parts:    a) On or about 12/7/2015; Punch press department, the northwest corner of the shop; the employer allowed employees to operate the Central Machinery belt sander without guarding the nip point on the belt; thus exposing employees to amputation  hazards.    b) On or about 12/7/2015; Punch press department, the northwest corner of the shop; the employer allowed employees to operate the Service Machine - Roussell # OA (1725 motor RPM) without guarding the nip point on the flywheel; thus exposing employees to amputation  hazards.    c) On or about 12/7/2015; in the west end of the shop; the employer allowed employees to operate the Milling and Drilling machine (Model 105-1300; Serial #344011) without guarding the rotating part of the chuck; thus exposing employees to struck-by hazards.

29 CFR 1910.217(c)(1)(i): The employer did not provide and ensure the usage of point of operation guards or properly applied point of operation devices on every operation performed on mechanical power press(es)    On or about 12/7/2015; Punch press department, the northwest corner of the shop; the employer allowed employees to operate the BenchMaster power presses to punch strappings without any point of operation guarding; thus exposing employees to amputation  hazards.

29 CFR 1910.37(b)(7): Each exit sign did not have the word "Exit" in plainly eligible letters not less than six inches (15.2 cm) high, with the principal strokes of the letters in the word "Exit" not less than three fourths of an inch (1.9 cm) wide:  On or about 12/7/15; in the southwest corner and the south center of the shop;e the employer allowed employees to work in the shop and the "exit" words were broken out of the light fixtures.
