29 CFR 1926.501(b)(1): Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level shall be protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. On or about December 16, 2015, roofers were observed preforming construction activities on the roof and near unprotected edges of a commercial building without fall protection or a written on-site fall protection plan. The employees were exposed to fall hazards at heights of greater than 6 feet above the ground.

29 CFR 1926.1053(b)(1): Where a portable ladder was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder. On or about December 16, 2015, the employer did not ensure a portable ladder extended at least 3 feet above landing surface or was secured at its top to provide assistance to the employees in mounting and dismounting. The employees were exposed to fall hazards at heights of greater than 6 feet above the ground.
