(b) Along the production floor, Line A, Ohtake noodle machine, Cutter area, on or about December 21, 2015, and at times prior thereto the employer did not provide any type of energy control procedures exposing employees to amputation hazards, when bypassing a guard during operation, and reaching into the roller section to clean product build up.
