29 CFR 1910.147(d)(2): The machine or equipment was not turned off or shut down using the procedures established for the machine or equipment: a) On December 22, 2015, on the second-floor mezzanine of the mail sorting room, a guard on an operating chain drive conveyor belt system was removed, without shutting down the equipment and following energy control procedures, exposing employees to amputation/caught in hazards.
