29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms or service rooms were not kept clean and orderly or in a sanitary condition:    Manufacturing Area of Facility: On or about December 22, 2016, employees were exposed to explosion, deflagration and/or fire hazards as a result of the accumulation of combustible dust on horizontal and vertical surfaces, where electrical junction boxes, ducts, and structural members of the facility were covered by combustible saw dust measuring from approximately 1/16 inch to 1-3/4 inches.
