29 CFR 1910.29(f)(4)(i): Units having more than five (5) steps or 60 inches vertical height to the top step were not equipped with handrails:    a)     On or about December 18, 2015 and at times prior, the employer, Rocky Mountain Metal Container, LLC, did not ensure employees, who were required to climb steps to a mobile ladder platform approximately 70 inches in vertical height for servicing operations on the Concord C24 Printer were protected from falling hazards due to a lack of step handrails.        Abatement Note:  Abatement certification and documentation is required for this item (see enclosed Sample Abatement-Certification Letter).

29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:  a)     On or about December 18, 2015 and at times prior, the employer, Rocky Mountain Metal Container, LLC, did not use utilize energy control procedures and therefore, did not ensure employees, who were required to engage in troubleshooting operations, including the removal of stuck cans on the Concord C24 Printer, were protected from hazards associated with the unexpected energizing and startup of system equipment or machinery. 29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    a)     On or about December 18, 2015 and at times prior, the employer, Rocky Mountain Metal Container, LLC, did not ensure employees, who were required to engage in printing and production operations on the Concord C24 Printer were protected from caught-in and/or crushing injuries due to a malfunctioning interlock on the Transfer Disc housing.        b)     On or about December 18, 2015 and at times prior, the employer, Rocky Mountain Metal Container, LLC, did not ensure employees, who were required to engage in printing and production operations on the Concord C24 Printer, were protected from caught-in and/or crushing injuries due to unguarded inker rollers.
