29 CFR 1910.147(c)(7): The employer did not provide training to ensure that the purpose and functions of he energy control program are understood by employees and that the knowledge and skills required for the safe application, useage, and removal of the energy controls are acquired by employees.  The training shall include:  (A) Each authorized employee shall receive training in the recognition of the applicable hazarrdous energy sources, the type and mangnitude of the energy available in the workplace, and the methods and means necessary for energy isoltion and control.  (B) Each affected employee shall be instructed in the purpose and use of the energy control procedure.  (C) All other employees shall be instructed about the procedure, and about the prohibition relating to attempts to restart or reenergize machines or equipment which are locked or tagged out:    (a) Facility:  On or about December 21, 2014, two (affected) employees were not trained in the company's lockout/tagout program.

29 CFR 1910.303(b)(7): 29 CFR 1910.303(b)(7):  Electrical equipment was not installed in a neat and workmanlike manner:    Facility:  On or about December 21, 2015, the Niagara 6 foot shear had its on/off switch not working and the company was using the machines electrical disconnect as its on/off switch.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:     (a) Facility:  On or about December 21, 2015, the Plexiglas shear guard to the 6 foot Niagara had a minimum opening of  1 1/16 inches and the shear point was only 3 3/4 inches from the shear guard.
