The equipment had been installed approximately 2 to 3 months prior to an employee receiving an amputation injury to right hand.

An employee received an amputation to right hand due to not locking the equipment out before performing maintenance or cleaning of the machine.

29 CFR 1910.1200(j)(1): The employer did not train employees regarding the new label elements and safety data sheets format by December 1, 2013. 1) Production Area - The employer did not provide training to all employees on the new hazard communication standard Global Harmonized System (GHS) that took effective December 1, 2013. Employees were not familiar with the new hazard communication standard. Employees are using hazardous chemicals such as but limited to Carbon dioxide, Acetone, Petroleum Base and Propane.
