29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes: a) On or about 12/22/2015 - 5000 Pulaski Pike Northwest, Huntsville, AL, Building #4, employees installing 6-inch PVC pipe for the roof drains were exposed to fall hazards of 14 feet due to open holes.
