29 CFR 1910.147(d)(2): The machine or equipment was not turned off or shut down using the procedures established for the machine or equipment: The employer is failing to ensure employees are protected from hazards created by the energization or the unexpected start up of machines during set-up or maintenance activities. On or about, December 15, 2015, an employee working at 13027 Renfro Circle, Omaha, NE sustained an amputation injury due to equipment not being shut down during set-up operations. Tyson Foods, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29CFR 1910.147(d)(2), which was contained in OSHA inspection number 902235, citation number 1, item number 1b and was affirmed as a final order on January 27, 2015; with respect to a workplace located at 5200 Dakota Avenue, Dakota City, NE 68731.
