29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii) On or about December 23, 2015, at the above addressed jobsite, an employee worked from a mobile scaffold up to approximately 12 feet high without the working platform being fully planked, exposing the employee to a fall hazard.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, sturcure, personnel hoist, or similar surface was not used. Crossbraces were used as a means of access: On or about December 23, 2015, at the above addressed jobsite, an employee accessed a mobile scaffold by climbing the rails and not utilizing a ladder, exposing the employee to a fall hazard of approximately 12 feet.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: On or about 23 December 2015, at the above addressed jobsite, employees utilized an aluminum extension ladder to access the exterior scaffold without extending the ladder at least 3 feet above the landing surface, exposing the employees to a fall hazard of approximately 13 feet.
