Toledo, OH: The employer did not have a plan or program to identify job site hazards which assists employees in maintaining a safe working environment. Such programs would support the employer and employees in identify and avoiding potential hazards, such as falls during job site operations, decreasing the chance of employee injury and/or illness.

29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section: a) Edinson Sterling DBA Sterlings Home Improvement on or about December 22, 2015, at the address of 521 Reynolds Rd. Toledo, OH: The employer failed to ensure an employee was protected from a fall while installing plastic wind screens around a supported scaffolding system. The employee tasked with this work was operating (standing) atop the second walk through frame at the unguarded north end of the scaffold system. The employee working in this condition was exposed to a fall of approximately 14 ft. to the pavement below.
