29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, sturcure, personnel hoist, or similar surface was not used. Crossbraces were used as a means of access. Jobsite: Ladder jack scaffold on east side of building. Employees were exposed to fall hazards of about 15 feet due to the improper placement of the jack and the lack of a separate ladder to access the work platform.

29 CFR 1926.451(g)(1)(i): Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold was not protected by a personal fall arrest system: Jobsite: East side of building An employee on a ladder jack scaffold was not 'tied off' and was exposed to fall hazard of about 15 feet from the scaffold work platform to the ground below.
