29 CFR 1910.147(f)(1)(i): 29 CFR 1910.147(f)(1): WHEN LOCKOUT OR TAGOUT DEVICES WERE TEMPORARILY REMOVED FROM THE ENERGY ISOLATING DEVICE AND THE MACHINE OR EQUIPMENT WAS ENERGIZED TO TEST OR POSITION THE MACHINE, EQUIPMENT OR COMPONENT THEREOF, THE SEQUENCE OF ACTIONS IN 29 CFR 1910.147(f)(1)(i) THROUGH (f)(1)(v) WERE NOT FOLLOWED: Packing Room: A worker did not ensure that the guards of the machine were closed prior to jogging the machine to check the operation of the suction cups that grab paperboard. A worker was injured.

29 CFR 1910.147(f)(1)(ii): When lockout or tagout devices temporarily were removed from the energy isolating device and the machine or equipment was energized to test or position the machine, the employees were not removed from the machine or equipment area in accordance to the paragraph (e)(2): Packing Dept.: A worker did not remove other employees from the Kelpac box forming machine prior to jogging the machine. A second worker broke her arm.
