29 CFR 1910.147(c)(4)(ii): The energy control procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, including, but not limited to Items of this section:    (a) On or about December 16, 2015, the employer did not ensure that lockout/tagout procedures for the "C-3840 Mold Closer" machine specifically outlined the scope, purpose, authorization, rules, and techniques to control hazardous energy. This exposed employees to amputation, caught-in-between, and struck-by hazards.

29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed:    (a) On or about December 16, 2015, the employer did not ensure that a periodic inspection of the energy control procedures were conducted for the C-3840 mold closing machine.

29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6):    (a) On or about December 16, 2015, employees were exposed to machine hazards associated with moving parts and gravity when they engaged in alignment activities on the closing machine. The employer did not ensure that employees implemented energy control application steps prior to allowing them to conduct routine adjustments of the drag mold. The machine was not shut down or locked out prior to engaging in this servicing and maintenance activity. As a result, the remaining applicable energy control elements, involving machine isolation [1910.147(d)(3)], lockout/tagout device application [1910.147(d)(4)], dissipation of residual energy [1910.147(d)(5)(i)], and verification of isolation [1901.147(d)(6)] were not implemented to protect employees from struck-by/crushing hazards.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    (a) On or about December 16, 2015, the employer did not ensure that adequate guarding was utilized around the "C-3840 Mold Closer" machine in that employees can open a yellow gate directly to the west of the "closing machine" and have access to moving equipment such as, but not limited to, the cope and drag molds.     (b) The employer did not ensure that guarding was  utilized behind the "C-3840 Mold Closer" machine where the cope and drag molds exited the machine. Employees can enter on to the mold boards while the machine is operational to access the control panel in the back of the machine.     (c) The employer did not ensure that adequate guarding was utilized around the south side of the "C-3840 Mold Closer" machine in that employees can walk on a platform in close proximity to the drag and cope; and when employees have to reach on top of the closing components to activate a sensor switch.
