29 CFR 1910.134(f)(2): Employee(s) using tight-fitting facepiece respirators were not fit tested prior to initial use of the respirator, whenever a different respirator facepiece was used, and at least annually thereafter: a) Gas Main Break at Stanton Street and Hazle Street in Wilkes-Barre, PA: An employee, Utility A, was required to wear a MSA Ultra Elite full-face respirator while performing gas main break repair activities and the employer did not provide this employee with a fit test prior to his initial use of the respirator, discovered on or about 12/24/2015. b) Gas Main Break at Stanton Street and Hazle Street in Wilkes-Barre, PA: An employee, Mechanic II, was required to wear a MSA Ultra Elite full-face respirator while performing gas main break repair activities and the employer did not provide this employee with an annual respirator fit test within one year of his previous fit test, discovered on or about 12/24/2015. ABATEMENT CERTIFICATION REQUIRED
