The following machines are not de-energized prior to servicing: a/ Inside of the Production Area, the series of augers belonging to the Breader Machines on Lines #1, #2, #3a, and #3b are not being de-energized prior to removing breading material. A multiple amputation occurred on or about 12/22/15. b/ Inside of the Production Area, the ILA-Pak Bagger, m/n OF2000, s/n 074111001, is not being de-energized prior to clearing a jam. A amputation occurred on or about 01/23/16. c/ Inside of the Production Area, specifically Line #2, the Breader Applicator, m/n 0045, is not being de-energized prior to cleaning the inadequately guarded auger blades.

Inside of the Production Area, the mixer blades belonging to the Extruder Machine, Shell #125, Jacket #0625, are inadequately guarded in that the cover could be lifted up without the use of a hand tool or the equivalent. b/ The employer is failing to protect employees from amputation hazards. This condition was last observed on and about 02/10/16, at 2 Industrial Drive, Piedmont, MO 63957. Inside of the Production Area, specifically Line #1, the auger belonging to the Batter Machine, m/n BR0010, is inadequately guarded. The opening directly above the auger measures approximately 4" X 4". c/ The employer is failing to protect employees from amputation hazards. This condition was last observed on and about 02/10/16, at 2 Industrial Drive, Piedmont, MO 63957. Inside of the Production Area, specifically Line #2, the auger belonging to the Breader Applicator, m/n 0045, is inadequately guarded. The opening approximately six inches above the auger blades measures approximately 3" X 3". Windsor Foods was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 29 CFR 1910.212(a)(1), which was contained in OSHA inspection number 899184, citation number 1, item number 1, and was affirmed as a final order on 04/29/13, with respect to a workplace located at 2 Industrial Drive, Piedmont, MO 63957.
