29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: On or about November 11, 2015 an employee unjamming the Viking 504 is exposed to a caught by hazard when the stringer caused the machine to jam up to thirty (30) times a day. The equipment was not locked out.
