29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a. Site- Employees operating the SWI TRAK DPMS2 vertical milling machine are not protected from amputation, laceration, and fracture injuries where the point of operation and rotating parts are not guarded from contact. On or about December 17, 2015 an employee sustained injuries when they contacted a metal rod that was being cleaned with emery paper while the rod was inserted into the chuck of the milling machine and turned. The paper was placed against the rod while it was turned by the milling machine without any barrier guards.
