29 CFR 1926.404(f)(6): The path to ground from circuits, equipment, or enclosures was not permanent and continuous. a) Roof level: Employees used electric powered hand tools to do masonry work. The tools were connected to extension cords, which were routed to a series of extension cords that were routed to outlet devices in the basement. The extension cords were missing ground pins. On or about 12/11/15.
