29 CFR 1926.25(a): During the course of construction, alteration, or repairs, form and scrap lumber with protruding nails, and all other debris, was not kept cleared from work areas, passageways, stairs, in and around buildings or other structures.  Location:  East side of the North portion of the building, 486 Oxford Road, Oxford CT, 06478.  Roofing material debris and 2 1/2 inch nails protruding from the perimeter/edges of a blue tarp lying on the ground used to collect the roofing material debris was not clear from work areas and passageways.

29 CFR 1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns:  Location: East side of the North portion of the building,  486 Oxford Road, Oxford CT, 06478.  Protective helmets were not worn in working areas underneath the roof where tools and other objects on the roof had the possibility of causing head injuries.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  Location: 486 Oxford Road, Oxford CT, 06478.  The employer did not ensure that the employees who were exposed to fall hazards while working from the roof of the municipal building at heights above six feet were trained in fall hazards and the procedures to be followed to minimize such hazards.

29 CFR 1926.102(a)(1): The employer does not protect employee(s) engaged in using machines or operations which present potential eye or face injury from physical, chemical, or radiation agents.  This violation was most recently observed on 12/23/2015 at a worksite at 486 Oxford Road, Oxford, CT where eye and face protection was not provided where physical agents (pneumatic nail guns) were utilized in the workplace.     Paucar Roofing & Siding LLC has been previously cited for this practice as described below.   Paucar Roofing & Siding, LLC was previously cited for a violation of this occupational safety and health standard 29 CFR 1926.102(a)(1) which was contained in OSHA inspection number 901341, citation number 01, item number 001, affirmed as final order on 6/28/2013 with respect to a workplace located at 305 Main Street, Watertown, CT.   To abate this violation, the employer must ensure that eye and face protection is available and utilized by each employee on its worksites to protect each employee working with pneumatic nails exposed to eye or face hazards.  Pursuant to 29 CFR 1903.19(d), the employer must provide documentation that this violation has been abated within 10 calendar days it receives the citation. 29 CFR 1926.501(b)(11): The employer does not protect employees(s) engaged in residential construction activities 6 feet or more above a lower level from falling to that lower level.    This violation was most recently observed on 12/23/2015 at a worksite at 486 Oxford Road, Oxford, CT where guardrail systems, safety net systems or personal fall arrest systems were not provided on a 6 in 12 pitch roof exposing employees to falls from elevation up to 21 feet.     Paucar Roofing & Siding LLC has been previously cited for this practice as described below.   Paucar Roofing & Siding, LLC was previously cited for a violation of this occupational safety and health standard 29 CFR 1926.501(b)(11) which was contained in OSHA inspection number 901341, citation number 01, item number 002, affirmed as final order on 6/28/2013 with respect to a workplace located at 305 Main Street, Watertown, CT.    To abate this violation, the employer must ensure that all equipment necessary to protect employees against falls from roofs is available at its worksite and that such equipment is used to protect all employees who work on roofs more than 6 feet above a lower level from falling to that lower level.  Pursuant to 29 CFR 1903.19(d), the employer must provide documentation that this violation has been abated within 10 calendar days it receives the citation.
