29 CFR 1910.178(l)(4)(iii): An evaluation of each powered industrial truck operator's performance was not conducted at least once every three years:    a. Facility - On or about December 25, 2015, an employee was operating a powered industrial vehicle to move around packages and had not been evaluated or retrained in this vehicle since 2002.    The U. S. Postal Service was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR 1910.178(l)(4)(iii), which was contained in OSHA inspection number 97272, citation number 2, item number 1, and was affirmed as a final order on September 20, 2011, with respect to a workplace located at 1207 Sawmill Creek Road, Sitka, Alaska.    The U. S. Postal Service was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR 1910.178(l)(4)(iii), which was contained in OSHA inspection number 110843, citation number 1, item number 3, and was affirmed as a final order on March 7, 2012, with respect to a workplace located at1750 Lundy Ave, San Jose, California.    The U. S. 29 CFR 1910.120(q)(8)(i): The employer did not ensure that employees who are trained in accordance with paragraph (q)(6) of this section received annual refresher training of sufficient content and duration to maintain their competencies, or demonstrated competency in those areas at least yearly.   a. Facility - On or about January 21, 2016, the employer did not ensure that an employee assigned to respond to potential emergency situations had received annual refresher training.

29 CFR 1910.1030(g)(2)(iv):  Annual training for employees with occupational exposure was not provided within one year of their previous training:         a.  Facility- On or about 15 December, 2015, an employee had not received annual training on bloodborne pathogens and assisted in the response to a potential bloodborne pathogen-contaminated parcel.      Abatement note: After the initial training, the employer shall provide annual training for all employees within one year of their previous training.  The training shall be provided at no cost to the employee and during working hours.  The employer shall ensure employee participation in the training program.  Training records shall include the information described in 1910.1030(h)(2)(1)(A) through (D); and these records shall be maintained for 3 years from the date on which the training occurred.
