29 CFR 1910.333(a): Safety-related work practices were not employed to prevent electric shock or other injuries resulting from either direct or indirect electrical contacts, when work was performed near or on equipment or circuits which were or could be energized: On or about December 29, 2015, The employer failed to ensure employees performing work within an industrial electrical panel box were protected from electrical hazards including arc flashes. The equipment was not deenergized prior to the employees working on or near the panel. The equipment contained live electrical hazards of 480 volts.

29 CFR 1910.332(b)(3): Additional requirements for qualified persons. Qualified persons (i.e. those permitted to work on or near exposed energized parts) shall, at a minimum, be trained in and familiar with the following: On or about December 29, 2015, the employer did not ensure that qualified employees were trained in safety-related work practices regarding deenergizing of equipment, application of locks and tags, verification of deenergized condition, and reenergizing equipment.
