29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) For employees working at 6212 29th Street East in Bradenton, as observed on or about December 30, 2015. Employees were operating Hardinge SG-42 computer numerical controlled (CNC) lathes, S/N SGA-1817-B and SGA-1832-B, with the gate safety interlock switches bypassed.

29 CFR 1910.303(b)(1)(iv): Electric equipment was not free from recognized hazards that were likely to cause serious physical harm to employees, such as damaged electrical insulation: a) For employees working at 6212 29th Street East in Bradenton, as observed on or about December 30, 2015. Employees were operating a company manufactured tip cleaning machine through the use of a flexible cord that had damaged outer electrical insulation.
