29 CFR 1926.651(b)(3): When excavation operations approach the estimated location of underground installations, the exact location of the installations was not determined by safe and acceptable means. Location: 240 White Street, Danbury, CT 06810 A safe and acceptable means in order to location energized underground electrical cables encased in concrete was not utilized. Consequently, while utilizing pneumatically powered air tools energized electrical cables to contacted and an electrical arc resulted injuring an employee.

29 CFR 1926.651(k)(1): Daily inspections of excavations, the adjacent areas, and protective systems were not made by a competent person for evidence of a situation that could result in possible cave-ins, indications of failure of protective systems, hazardous atmospheres, or other hazardous conditions. An inspection was not conducted by the competent person prior to the start of work and as needed throughout the shift. Location: 240 White Street, Danbury, CT 06810 An inspection of the excavation where underground energized electrical cables were encased in concrete was not inspected by a competent person for electrical hazards prior to utilizing pneumatic air tools to break apart the encased concrete.
