29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:  a. On the 6/12 pitched roof, fall protection was not provided to employees installing ISO roof decking, exposing the employees to a 16 foot 6 inch fall hazard, on or about 12/29/2015.
