At the jobsite, the single piece of scaffold planking an employee was working from while installing fascia boards was 12 inches wide, exposing the employee to a 12 foot 4 inch fall hazard, on or about 12/30/15.

At the jobsite, fall protection was not provided to an employee installing fascia boards while working from a single plank of scaffolding, exposing the employee to a 12 foot 4 inch fall hazard, on or about 12/30/15.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a. Along the exterior wall, the blue portable extension ladder used to access the roof extended approximately 24 inches or 1-1/2 rungs past the landing surface, exposing employees to a 12 foot 4 inch fall hazard, on or about 12/30/15.   b. Along the exterior wall, the orange portable extension ladder used to access the roof trusses did not extend past the landing surface, exposing employees to a 12 foot 4 inch fall hazard, on or about 12/30/15.
