29 CFR 1910.134(e)(1): The employer did not provide a medical evaluation to determine the employee's ability to use a respirator, before the employee was fit tested or required to use the respirator in the workplace:  Seal It Insulation Systems -   On or about December 22, 2015, the employer did not provide a medical evaluation for the employees who, on a mandatory basis, used the full face, air supplied respirator and half face fitted respirator when applying isocyanates (such as but not limited to 1,6-Hexamethylene Diisocyanate Homopolymer, Methylene Bisphenyl Isocyanate, and Hexamethylene Diisocyanate) in the workplace.

29 CFR 1910.1200(h)(3)(iii): Employee training did not include the measures employees can take to protect themselves from chemical hazards, including specific procedures the employer had implemented to protect employees from exposure to hazardous chemicals, such as appropriate work practices, emergency procedures and personal protective equipment to be used:  Seal It Insulation Systems -  On or about February 10, 2016, employees were not provided with effective training and information regarding the isocyantes (such as but not limited to 1,6-Hexamethylene Diisocyanate Homopolymer, Methylene Bisphenyl Isocyanate, and Hexamethylene Diisocyanate) used in the workplace including measures to take to protect themselves from exposure to these hazards (for example, hand protection and specific air respirator cartridges).
