29 CFR 1910.23(a)(8): Every floor hole into which persons can accidentally walk were not guarded: (a) In the break room, the employer did not ensure that a floor hole, located between the refrigerator and lockers, was protected with a securely held cover to prevent anyone from accidentally stepping into the hole.

OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to fall hazards: Among other methods, recognized and feasible means of abatement to correct these hazards include, but are not limited to providing approved equipment designed for personnel to safely dump the cart into the dumpster. Abatement verification is required for this violation. The abatement certification sheet is enclosed with the citation(s).

29 CFR 1910.305(g)(2)(ii): Flexible cords were not used only in continuous lengths: (a) The employer failed to ensure that flexible cords were used only in continuous lengths. In the storage and data room, a TrippLite Isobar Ultra 6-Outlet Surge Suppressor protector and a TrippLite Protector multiple receptacle Relocatable Power Tap were connected together to supply power to data communications center. This condition exposed employees to fire hazards.
