This condition exposed employees to amputation hazards.

29 CFR 1910.134(h)(1): Respirators were not cleaned and disinfected using the procedures in Appendix B-2 of 29 CFR 1910.134 or equivalent procedures recommended by the respirator manufacturer: (a) Delta Rigging and Tools at 722 Scarlet Drive, Grand Junction, CO 81501: On or before December 17, 2015, the employer did not ensure that each respirator user was provided with a respirator that is clean, sanitary, and in good working order. At the time of inspection, respirators were not being properly maintained, in that they were stored next to a machine grinder, thus exposed to potential contamination. This condition potentially exposed employees to respiratory hazards.
