29 CFR 1910.134(f)(1): The employer did not ensure that employee(s) required to use a tight-fitting facepiece respirator passed the appropriate qualitative fit test (QLFT) or quantitative fit test (QNFT): a) Coal Dock Area: On or about January 04, 2016 and at times prior, employees were required to wear 3M half mask negative pressure air purifying respirators with combination cartridges without being fit tested prior to use.

29 CFR 1910.134(h)(2)(i): Respirators were not stored to protect them from damage, contamination, dust, sunlight, extreme temperatures, excessive moisture, and damaging chemicals or were not packed or stored to prevent deformation of the facepiece and exhalation valve: a) Lime Kiln Area: On or about January 4, 2016 and at times prior, employees were exposed to calcium carbonate and calcium oxides resulting from improperly storing a respirator on the desk. b) Coal Dock Area: On or about January 4, 2016 and at times prior, employees were exposed to coal dust resulting from storing respirators from a hook in the Control Room for the Coal House.
