29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with paragraph (b) or (c) of this section: (a) Jobsite, Route 8, Middlesex Township, PA: On or about December 30, 2015 employees who entered the excavation that was approximately 6 feet in depth with near vertical side walls were not protected from cave-ins by adequate protective systems, such as but not limited to: protective shields or benching.
