29 CFR 1910.106(e)(6)(i): Adequate precautions were not taken to prevent the ignition of flammable vapors:    (a) Mills Solids Control Consulting @ Aggregate State /37C-16HZ, Fort Lupton, CO:  On and prior to 12/13/2015, the employer had not ensured precautions were taken to prevent the ignition of flammable vapors.  On 12/13/2015, an employee was seriously burned while using an open flame propane weed burner to thaw a frozen Fisher brand liquid transfer pump. The open flame weed burner ignited hydrocarbon vapors emanating from the hazardous product inside a three-sided container with high walls measuring approximately 15 feet wide x 30 feet long X 8 feet high.

29 CFR 1910.151(c): Where employees were exposed to corrosive liquid materials, suitable facilities for quick drenching or flushing of the eyes and/or body were not provided within the work area for immediate emergency use:     (a)  Mills Solids Control Consulting @ Aggregate State /37C-16HZ, Fort Lupton, CO:  On and before 12/13/2015, Mills Solids Control Consulting did not ensure that where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and/or body were provided within the work area for immediate emergency use.  Employees used a chemical product with a pH of 2 (CWP99), while working inside the water treatment trailer.  This condition exposed employees to a chemical burn hazard.     Abatement Note:  For details on minimum requirements for a suitable emergency eyewash and shower equipment, OSHA references the American National Standard for Emergency Eyewash and Shower Equipment, ANSI Z358.1

On 12/13/2015 an employee was seriously burned after hydrocarbon vapors ignited inside the container with high walls.

On 12/13/2015, an employee was seriously burned while using an open flame propane weed burner to thaw a frozen Fisher brand liquid transfer pump. The open flame weed burner ignited hydrocarbon vapors emanating from the hazardous product inside the container with high walls.

Employees were not trained to utilize devices such as 4 gas monitors for the detection of hydrocarbons prior to using an open flame propane weed burner to thaw a frozen Fisher brand liquid transfer pump.  An employee received 2nd and 3rd degree burns after flammable vapors ignited inside the container with high walls.
