29 CFR 1926.502(d)(15): Anchorage used for attachment for personal fall arrest equipment was not capable of supporting at least 5,000 pounds (22.2 kN) per employee attached nor was it designed, installed and used as follows: As part of a complete personal fall arrest system which maintains a safety factor of at least two; and under the supervision of a qualified person.   On or about January 5, 2016, an employee was exposed to 25 foot fall hazard while using a fall protection anchor that was not installed per the manufacturer's specifications and would not support 5,000 pounds.   Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).

29 CFR 1926.501(b)(10):     "Roofing work on Low-slope roofs." Except as otherwise provided in paragraph (b) of this section, each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels shall be protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system. Or, on roofs 50-feet (15.25 m) or less in width (see Appendix A to subpart M of this part), the use of a safety monitoring system alone [i.e. without the warning line system] is permitted.    On or about January 5, 2016, employees were performing work on a low-slope roof without a sufficient safety monitoring system or other method of fall protection.

29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder.        On or about January 5, 2016 employees used an "A" frame ladder to access an upper landing.  The ladders length did not allow for three feet of extension past the point of access, exposing employees to the hazards associated with a fall.      Apex Exteriors, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.1053(b)(1), which was contained in OSHA inspection number 910870, citation number 1, item number 1 and was affirmed as a final order on July 23, 2013, with respect to a workplace located at 39W590 Mathewson Geneva, Illinois 60134.      Apex Exteriors, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.1053(b)(1), which was contained in OSHA inspection number 314735648, citation number 01, item number 04 and was affirmed as a final order on October 21, 2010, with respect to a workplace located at 2408 Ridgeway Ave. Evanston, Illinois 60202.        Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d).
