29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling: (a) Workstation 1-4; A relocatable power tap (surge protector) was plugged into an extension cord, because the cord was too short. The UL listing states that relocatable power taps are designed to be plugged into an outlet, and not daisy chained and plugged into other relocatable power taps or extension cords. This exposes employees to possible electrical fire hazards. (b) Workstation 1-7; An extension cord was plugged into a relocatable power tap (surge protector). The UL listing states that extension cords cannot be plugged into other extension cords or relocatable power taps, this is considered a daisy chain . This exposes employees to possible electrical fire hazards. (c) Workstation 1-10; A white extension cord has been used to power a fan and computer equipment since April 2015. The UL listing states that extension cords are only to be used temporarily, 90 days or less. This exposes employees to possible electrical fire hazards.
