29 CFR 1910.178(n)(6): Industrial truck drivers were not required to look in the direction of, and keep a clear view of the path of travel: (a) On or about December January 4, 2016, employee was exposed to struck-by hazards at the loading ramp behind J.R. Butler buiding, due to the operator of a powered industrial truck not staying in constant communications with tag line operator. The forklift operator did not keep clear view of path of travel. The tag line operator was struck-by a powered industrial truck driven by an operator, while traveling with a load. Abatement Note: Abatement certification is required for this item (see enclosed "Certification of Corrective Action Worksheet").
