29 CFR 1910.132(f)(3)(iii): The employer did not retrain employees when inadequacies in the affected employee's use of PPE indicated the employee had not retained the requisite understanding. Location: Machine shop On or about 12/14/15, an employee operating an Alliant-RT2, serial #70410719 manual vertical milling machine with gloves amputated the left thumb and received lacerations to the left index finger.
