29 CFR 1910.23(e)(2): Stair rails did not consist of a top rail, intermediate rail and/or posts, with the vertical height not more than 34 inches (86.36 cm) nor less than 30 inches (76.2 cm) from upper surface of top rail to surface of tread in line with face of riser at forward edge of tread:  A. On or about January 7, 2016, Rear Stairway to Mezzanine:  The stair rails were not provided with an intermediate rail, exposing employees to fall hazards.

29 CFR 1910.37(a)(3): Exit routes were not kept free and unobstructed.  Materials or equipment were placed, either permanently or temporarily, within the exit route:  A.  On or about January 7, 2016, Receiving Area:  Access to the identified emergency exit door was blocked by the storage of two wrapped pallets of materials and the Komatsu F1315MU battery-powered fork truck.  Access was reduced to twelve inches on one side of the pallets and sixteen inches on the other side.

29 CFR 1910.37(b)(5): Each doorway or passage along an exit access that could be mistaken for an exit was not marked "Not an Exit" or similar designation or identified by a sign indicating its actual use:  A.  On or about January 7, 2016, Shop Area:  The side-hinged exterior door at the back of the building was not provided with a sign indicating it was not intended to be used as an emergency exit.  The door was locked during building occupancy with slide-bolt locks at the top and bottom of the door.
