29 CFR 1910.219(d)(1): Pulley(s) with part(s) seven feet or less from the floor or work platform were not guarded in accordance with the requirements specified in 29 CFR 1910.219(m) and (o): (a) Plant Floor, Dry process East End, employees were exposed to serious caught-in, crushing and or struck-by injuries, the Pulley System on the papermill machine did not have a guard that fully enclosed the moving parts of a pulley 75 to 83 inches from the ground on all exposed sides.

29 CFR 1910.219(e)(3)(ii): All guards for inclined belts shall be arranged in such a manner that a minimum clearance of seven (7) feet is maintained between belt and floor at any point outside of guard. (a) Plant Floor, Dry process East End, employees were exposed to serious caught-in, crushing and or struck-by injuries, the Pulley System on the papermill machine did not have a guard that fully enclosed the moving parts of a pulley 75 to 83 inches from the ground on all exposed sides.
