29 CFR 1910.107(g)(3): Residue scrapings and debris contaminated with residue were not immediately removed from the premises and properly disposed. Approved metal waste cans were not provided wherever rags or waste were impregnated with finishing material; and all such rags or waste were not deposited therein immediately after use. The contents of waste cans were not properly disposed of at least once daily or at the end of each shift:  PAINT MIXING ROOM  On, or about, January 5, 2016, approved metal waste cans were not provided. Rags impregnated with flammable materials, such as, but not limited to, ATX Spot Clear Coat (Part A), were used and disposed of into plastic waste bins.

29 CFR 1910.151(b): First aid supplies approved by a consulting physician were not readily available:   BAY AREA  On, or about, January 5, 2016, the employer failed to provide an approved antidote for hydrofluoric acid skin absorption, such as, but not limited to calcium gluconate, when employees worked with Key Extreme Wheel Cleaner, a corrosive cleaner that contained up to 10% ammonium bifluoride.

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area:  BAY AREA  On, or about, January 5, 2016, employees that worked with Key Extreme Wheel Cleaner, which contained ammonium bifluoride, had not been trained on the health hazards of the chemical. Ammonium bifluoride may be absorbed through the skin and cause internal, chemical burns.
