29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent) and toeboards:    a) GIMA machines in the Foundry area: On or about January 4, 2016 and at times prior thereto, the employer did not ensure a fall protection method was used while working on top of the machine which exposed employees to falls up to 12-feet.    b) IMT 4 Axis machines in the Machining area: On or about January 4, 2016 and at times prior thereto, the employer did not ensure a fall protection method was used while working on top of the machine which exposed employees to a fall of 9-feet.      c) Upper Conveyor in the Machining area: On or about January 4, 2016 and at times prior thereto, the employer did not ensure a fall protection method was used while working along conveyor which exposed employees to a fall of 26-feet.

29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control:     a) GIMA Machines: On or about January 4, 2016, the employer did not ensure Die Maintenance employees were trained on lockout requirements while conducting servicing related activities which exposed employees to amputations and other injures associated with being crushed-by moving machinery.      b) Robotic Cells: On or about January 4, 2016, the employer did not ensure Production Tech employees were trained on lockout requirements while conduct repair and/or servicing related activities in these cells which exposed employees to amputations and other injures associated with being crushed-by and struck-by moving machinery.
