Location: 1355-1359 Myrtle Ave, Brooklyn, NY  a) The Makita grinder and the Skilsaw circular saw used by an employees were powered by extension cords that were missing the ground pin.  The path to ground was not permanent and continuous; on or about 12/29/15.
