On or about December 31, 2015, employees making a permit required confined space entry into the #5 Sorbitol Reactor at the ADM Corn Plant were exposed to an oxygen-deficient atmosphere containing nitrogen and the employer did not implement the means and practices necessary to ventilate the permit space as necessary to eliminate or control the atmospheric hazard.
