29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:  a. Coventry Management, Inc. - Elyria, Ohio: On or about January 5, 2016, the employer did not develop and implement an energy control program that includes training, written procedures, and audits, to protect employees from hazardous energy sources while maintaining machines.  An employee sustained an amputation injury while servicing and unjamming the Auto Pak trash compactor (s/n 24FA3145L11) during a time the machine was not locked out.
