OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees:    a) 200 Bellport Ave, Medford, NY: An employee incurred crushing injuries while operating a lift gate on the back of a company vehicle; on, or about, 01/07/2016.      ABATEMENT NOTE: among other feasible means of abatement, the employer should implement the following measures:      1)    Obtain and ensure that employees who operate equipment read the Owners Manual before operating the lift gate;  2)    Do not operate the lift gate until if you do not have a thorough knowledge and understanding of the operation of the lift gate;   3)    Establish a training program for employees in the proper and safe use of lift gates;  4)    Ensure equipment operators keep hands and feet clear of all pinch points;   5)    Ensure electric equipment is installed in a neat and workmanlike manner and;   6)    Ensure electric equipment likely to be exposed to physical damage, is enclosed or guarded, and so arranged and of such strength as to prevent damage.
