29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative: A) Barns Unlimited, Inc. DBA 501 Metals, located at 5350 Wapak Road, Lima OH 45806: On or about January 6, 2016, the employer failed to establish a lockout/tagout program for the control of the hazardous energy, which includes procedures, training, and periodic inspections, while performing activities such as, but not limited to, conducting maintenance of the equipment such as, but not limited to, the MRS metal roll forming system (Serial # 02-01-100-T) and the Acu-Form slitter (Model SLI-4648, Serial # 0513-S18). Employees were exposed to amputation hazards.

DBA 501 Metals, located at 5350 Wapak Road, Lima OH 45806: On or about January 6, 2016, the employer failed to provide training and evaluation to ensure that employees, operating powered industrial vehicles such as, but not limited to, the Yale forklift (Model GLC120SBV096, Serial # N448635) were competent to safely complete the task. Employees were exposed to struck-by, caught-in-between, and tip-over hazards.

Employees were exposed to pinch point hazards while operating the panel former. B) Barns Unlimited, Inc. DBA 501 Metals, located at 5350 Wapak Road, Lima OH 45806: On or about January 6, 2016, the employer failed to ensure that the point of operation of the profile shear machine (data plate was not located) was adequately guarded. Employees were exposed to pinch point hazards while operating the profile shears machine.
