29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) Worksite, 55 Union Street, Oceanside NY: CEC Roadrunner Screening Machine 8x16, Tail Pulley Shaft with a keyway slot measuring approximately 40 1/2 inches from the walking surface to the centerline was not guarded; on or about 1/7/16. b) Worksite, 55 Union Street, Oceanside NY: CEC Roadrunner Screening Machine 8x16, Tail Pulley measuring approximately 40 1/2 inches from the walking surface to the centerline was not guarded between the ingoing nip point of the Conveyor Belt and the Tail Pulley; on or about 1/7/16. c) Worksite, 55 Union Street, Oceanside NY: CEC Roadrunner Screening Machine 8x16, Conveyor Belt Roller measuring approximately 68 inches from the walking surface to the centerline was not guarded between the ingoing nip point of the conveyor belt and the Conveyor Belt Roller; on or about 1/7/16. d) Worksite, 55 Union Street, Oceanside NY: CEC Roadrunner Screening Machine 8x16, Conveyor Belt Roller measuring approximately 48 inches from the walking surface to the centerline was not guarded between the ingoing nip point of the conveyor belt and the Conveyor Belt Roller; on or about 1/7/16. e) Worksite, 55 Union Street, Oceanside NY: CEC Roadrunner Screening Machine 8x16, Conveyor Belt Roller measuring approximately 61 inches from the walking surface to the centerline was not guarded between the ingoing nip point of the conveyor belt and the Conveyor Belt Roller; on or about 1/7/16. f) Worksite, 55 Union Street, Oceanside NY: CEC Roadrunner Screening Machine 8x16, Conveyor Belt Roller measuring approximately 55 inches from the walking surface to the centerline was not guarded between the ingoing nip point of the conveyor belt and the Conveyor Belt Roller; on or about 1/7/16. 29 CFR 1910.305(g)(2)(ii): Flexible cords were not used only in continuous lengths without splice or tap: a) Worksite, 55 Union Street, Oceanside NY: Overhead and alongside the Tail Pulley area of CEC Roadrunner Screening Machine 8X16 were multiple 220 volt power cords that supplied electrical power to the machine and equipment. The electrical cords were spliced; on or about 1/7/16. NOTE: In addition to abatement certification, the employer is required to provide abatement documentation for this item in accordance with 29 CFR 1903.19.
