29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    On or about September 3, 2015 the Haas CNC V-7 Mill machine was not equipped with guarding and did not have a safety/interlock switch. This condition exposed employees to the hazard of being struck-by or caught-in the machine spindle.

29 CFR 1910.1200(f)(7): The employer may use signs, placards, process sheets, batch tickets, operating procedures, or other such written materials in lieu of affixing labels to individual stationary process containers, as long as the alternative method identifies the containers to which it is applicable and conveys the information required by paragraph (f)(6) of this section to be on a label. The employer shall ensure the written materials are readily accessible to the employees in their work area throughout each work shift.    On or about January 7, 2015, the employer did not label the 6 black oxide vat stationary chemical tanks containing Sodium Metasilicate, liquid acid mixture for metal finishing, Distillates (petroleum) hydrotreated light, and Sodium Hydroxide. 29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:    On or about January 7, 2016, the employer did not establish a lockout tagout program for employees who repaired equipment such as but not limited to CNC Mill machines at the facility. This condition exposed employees to the hazards of being struck-by, caught in between rotating parts, and electrocution.

Within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, you must report the in-patient hospitalization, amputation, or loss of an eye to OSHA.    On or about September 3, 2015, an employee amputated his right middle finger on the Haas CNC V-7 Mill machine. This amputation was not reported to OSHA.
