29 CFR 1910.157(c)(4): Portable fire extinguishers were not maintained in a fully charged and operable condition: a. On December 16, 2015, the fire extinguisher on the portable trailer/cart, located in the yard of the establishment, was not maintained. The pin had been pulled and the seal had been broken. The needle on the gauge was showing less than fully charged fire extinguisher. Origin Energy American Samoa, Inc., was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.157(c)(4), which was contained in OSHA inspection number 952545, citation number 1, item number 3b was affirmed as a final order on 03/03/2014, with respect to a workplace located at main plant, American Samoa.
