29 CFR 1926.1053(b)(4):  Ladder(s) were used for purposes other than the purpose for which they were designed:        a) Garage Area - A 4 foot A-Frame stepladder ladder was provided for employee use for access and egress between two points without its metal spreaders or locking devices in the open position.      b) Second Story Hallway - An 8 foot A-Frame stepladder was provided for employee use for access and egress between two points of the building.        In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIV ACTION WORKSHEET).

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):          a) Employees were exposed to the hazard of falling approximately two stories while performing carpentry work from roof deck surfaces without fall protection.       Timberline Contractors LLC., was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501 (b)(13) which was contained in OSHA inspection number 977779, citation 1, item 2, and was affirmed as a final order on June 19, 2014, with respect to a worksite located at 616 Grant, Hinsdale, Illinois.         In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator was competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l):     29 CFR 1926.602(d): Note: The requirements applicable to construction work under this paragraph are identical to those set forth at paragraph 1910.178(l) of this chapter.        a) The employer was operating a JCB 550 rough terrain forklift without ensuring that he was competent to operate the equipment safely, as demonstrated by successful completion of forklift training and evaluation.     Timberline Contractors LLC., was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.178(l)(1)(ii) which was contained in OSHA inspection number 977779, citation 1, item 1a, and was affirmed as a final order on June 19, 2014, with respect to a worksite located at 616 Grant, Hinsdale, Illinois.
